The Nurse Case Managers/Diabetes Educators at CAHN help to provide patients with aggressive, individualized care to meet their specific diabetes, hypertension and cholesterol management needs. We work together with the patient’s Primary Care Provider to set realistic goals regarding diet, exercise and medication management. Our patients are then able to make lifestyle changes that enable them to take control of their own health maintenance.
